Indicator 5.4.1 · Engagement
Government Engagement
Political leadership is essential for driving ambitious climate action that protects health. Recognising the links between climate change and health in political statements and national climate plans can help place health at the centre of climate policy and decision-making. This indicator tracks engagement with climate change and health in national leaders’ statements at the United Nations General Debate and in Nationally Determined Contributions, which set out countries’ commitments for climate action.
Headline finding
The proportion of countries addressing climate change and health in UN General Debate statements fell for four consecutive years, from 66% in 2021 to 26% in 2025, although remaining above 2016 levels. By contrast, health remained prominent in national climate commitments: 93% of the 106 NDCs added to the UNFCCC registry in 2025, covering 131 countries, referenced health.

Government Engagement with Health and Climate Change
Number and proportion of speeches in the UN General Debate referencing health topics, climate change or climate-related topics, and both climate and health topics, from 1970 to 2025, by country, Human Development Index (HDI) group, World Health Organization (WHO) region, and Lancet Countdown region. For the combined climate-and-health measure, a speech is counted when at least one climate-change term occurs within ±25 tokens of at least one health term. Each speech is counted only once for each measure, regardless of the number of relevant mentions or co-occurrences.

About this indicator
Description
This indicator tracks government engagement in health and climate change in two key forums. It tracks mentions of climate change and health in statements made by national leaders at the United Nations General Debate, which is part of the annual United Nations General Assembly, as a proxy of high-level political engagement on these two topics as separate and related issues. It also tracks reference to health and climate change, as well as their prominence, in the text of all available Nationally Determined Contributions by Parties to the Paris Agreement (as of January 1st 2026).
Caveats
The results present a somewhat conservative estimate of high-level political engagement with the intersection of climate change and health. There may be examples of governments referring to climate change and health but not the direct linkages between the two and there may be examples of governments discussing the health impacts of climate change in their United Nations General Debate speeches but the distance between the climate change term and the health term exceeds 25 words. The analyses are based on a narrow range of search terms, which excludes reference to many of indirect links between climate change and health. NDC: Updates to NDCs and entirely new submissions are analysed as iterative versions without distinction. While the nature of updates may differ from new NDCs, this distinction does not affect the importance assigned to health within both types of documents.
